Commitment Deposit & Refund Policy

(出席保證金及退款政策)

To ensure community resources are fully utilized, we implement a Commitment Deposit system: （為確保社區資源得到充分運用，我們設有「出席保證金」機制：）

• Deposit Amount (保證金金額): $10 per session, totaling $30 for 3 sessions. （每堂 $10，3 堂合共 $30。）

• 100% Cash Refund (100% 現金全額退回): The full $30 cash will be refunded to you on the final day of the workshop, provided your child attends all 3 registered sessions. （只要小朋友上滿全數 3 堂活動，$30 現金將於最後一堂當天全數退還給家長。）

Terms and Liability Waiver

By submitting this form, I, the parent/guardian, agree to the following:

1. Liability: I understand that the organizers and volunteers will take all safety precautions, but shall not be held liable for any personal injury or property damage during the activity.

2. Medical Emergency: In the event of a medical emergency, I authorize the staff to seek professional medical treatment if I cannot be reached.

3. Media Release: I grant permission for photos/videos of my child to be taken during the workshop for church cultural archive and promotional purposes. (Please inform staff upon arrival if you wish to opt-out).
